Supported Living and Outreach
We provide essential support to individuals who need extra assistance to live more independently. By offering choices and opportunities, we help them learn new skills and achieve their personal goals and dreams.
Our approach is grounded in positive and proactive strategies, with a dedicated team working collaboratively to ensure the best outcomes for those we support.
We assist individuals with various conditions, including Autism Spectrum Disorder, mental health diagnoses, and other learning disabilities. Many of the people we support have previously faced restrictive practices, such as overuse of medication and limited access to specialist services. They may have experienced trauma and require time to recover and rebuild trust, making it crucial to consider their past experiences.
Each individual receives a bespoke support package tailored to their needs, including person-centred Behaviour Support Plans and skill targets. We have successfully supported people with complex behaviours to transition from intensive and restrictive settings to their own homes in the community. Through high-quality support and specialist input, we facilitate smooth transitions and significantly reduce placement breakdowns.
Independence and Quality of Life
Skill building is an essential element of our PBS Framework, enabling those in our care to achieve greater independence and a higher quality of life. Increased independence enhances dignity and broadens personal experiences.
Through person-centred planning and the PBS Framework, we closely engage with individuals in our care to understand them deeply. We participate in their activities, listen to their loved ones, learn their preferences, offer choices, and explore new experiences. Wherever possible, we involve them in decisions about their support, utilising adaptive technology and visual aids to facilitate communication.
At Community Living, we conduct thorough assessments of each individual’s behavioural patterns, strengths, and weaknesses to develop a personalised list of attainable skills, guiding them towards social independence over time.
We are passionate about witnessing the growth and achievements of those we support—whether it’s learning a new recipe, crossing the road safely, or experiencing their first haircut in the community. Using evidence-based assessments, we identify areas needing extra support and employ data-driven decision-making to monitor progress.
